Most recently at Deighton, I held operational ownership of a multi-instance Azure SQL estate supporting 500+ databases and 60+ TB of managed capacity, putting the right structure, visibility, ownership and controls in place before technical issues become business disruption.
Many operational failures aren't purely technical. They're structural. They come from gaps in ownership, visibility and decision-making. Over time, I saw the same pattern in my own career: real effort, real capability, but not enough structure around where it was going. The architecture was missing.
